A Phobia is a recurrent, excessive, irrational fear of a specific object or situation. The OCD Center of Los Angeles offers the following brief questionnaire in an effort to help you get a better idea of whether or not you are exhibiting signs of a phobia. Simply check those items that apply to you, and email it to us using the simple form below. While this questionnaire is not meant to replace a thorough evaluation, it may help in identifying a phobia.

I usually feel immediate, excessive anxiety when I am exposed to a specific object or situation.

 2. 

I usually feel excessive, unreasonable anxiety when I anticipate the possibility of being exposed to a specific object or situation.

 3. 

I can endure the anxiety I experience when I am exposed to the object or situation I fear, but only with a great amount of distress and discomfort.

 4. 

I am overwhelmed by the anxiety I experience when I am exposed to the object or situation I fear, and will almost always make a significant effort to avoid that phobic object or situation.

 5. 

The anxiety I experience before, during, and/or after exposure to the feared object or situation is interfering with my occupational or academic functioning, my social activities, and/or my relationships.

 6. 

I am afraid of spiders (Arachnophobia)

 7. 

I am afraid of snakes (Ophidiophobia)

 8. 

I am afraid of heights (Acrophobia)

 9. 

I am afraid of dogs (Cynophobia)

10.

I often touch, pick, and/or measure my perceived flaw.

I am afraid of thunder and lightning (Astraphobia, also known as Brontophobia, Tonitrophobia, or Ceraunophobia)

12.

I am afraid of needles and injections (Trypanophobia)

13.

I am afraid of enclosed spaces (i.e. elevators, closets, tunnels, small rooms, etc.) (Claustrophobia)

I am afraid of vomiting (by self or others) (Emetophobia)

15.

I am afraid of water (Hydrophobia)

16.

I am afraid of dental procedures (Dentophobia)

17.

I am afraid of insects (Entomophobia, also known as Insectophobia or Acarophobia)

I am afraid of having bad breath (Halitophobia)

I am afraid of the dark (Nyctophobia)

This questionnaire was developed partly on the basis of clinical experience of staff therapists at the OCD Center of Los Angeles, and partly as an adaptation of various lists, including The Phobia List and The A-Z List of Phobias. Please note that the above test is not meant to replace a complete and thorough evaluation by a licensed Cognitive-Behavioral therapist or other qualified mental health professional. Some people may benefit from medication, and may therefore benefit from a psychiatric evaluation. Likewise, a psychiatric assessment may be necessary to differentiate between a phobia and other psychological conditions. If an evaluation is indicated, the OCD Center of Los Angeles can refer you to a qualified psychiatrist in our area. Furthermore, it is imperative to make the distinction between phobias and other medical conditions. For this reason, a medical examination may be necessary.
